Reports this week stated that AC Milan decided against signing Chelsea defender Disasi following “technical assessments” made by head coach Massimiliano Allegri.
This leave the door open for Premier League rivals West Ham who are set to welcome the 27-year-old later this evening as he completes his medical.
Disasi spent the second half of last season on loan at Aston Villa after being slowly forced out the side by former manager Enzo Maresca. Since his departure, he's had little to no game time this season under new manager Liam Rosenior.
The Hammers recently made a bid for Southampton's Taylor Harwood-Bellis, which was rejected back by the Championship club.
West Ham are now set on the Frenchman who has made eight Champions League appearances in his career as well as 44 Premier League appearances as he aims to add that experience to manager Nuno Espirito Santo’s side.
The news was followed by an update from journalist Ben Jacobs, who confirmed the loan move is on.
