The 30-year-old had the chance to put his side 1-0 up from the penalty spot just before half time but put his effort well over Bernd Leno’s crossbar.
Man United are still without a win in the new Premier League season, albeit after just two games, sitting down in 16th.
Speaking to the press after the game, Fernandes revealed referee Kavanagh “triggered” him just before he was about to take his penalty.
He said: "I was upset. As a penalty-taker, you have your own routines, your own things that you do. It upset me because the referee didn't apologise.
“That is what triggered me in that moment, but that's not the excuse for missing the penalty. I had a very bad hit on the ball. I put my foot too under the ball, and that's why it ended up going over the bar."
